Anyone in New York and Pennsylvania can get vaccinated by pre-registration and appointment by filling out the Tops registration COVID 19 Vaccine form and sending it to our email.

Patients 12 years of age and older may be vaccinated.
To receive a free dose of the vaccine you must complete the registration form, which you can do at home by downloading and printing or at the vaccination center:

You will need the following information about yourself:
- Last name

After that, indicate the date of completion and sign a consent to provide your personal data and other necessary information.

Bring your ID card, health insurance, vaccine card (if it's a repeat vaccination) and medical records to your appointment.
Important! Keep in mind that the whole procedure will take at least 40 minutes.

You can get a booster shot if you have proper documentation that you received your last dose.
